Hi.

This sort of a thing is most likely caused by a bug in the application installer of the phone. You can try to remove the icon by reinstalling the application and then removing it again. Please try to install it to the same location as you did earlier (phone memory / memory card).

I personally had the same problem on my 3250, and couldn't get this icon to remove without having to do a deep reset to the phone. So I recommend that if a reinstall/remove doesn't help, simply take the phone to the retailer / Nokia service point to get it fixed.
